Google AI Overviews vs. Traditional Local SEO: What's Different for Med Spas

A lot of med spa owners assume that if their local SEO is in good shape, their AI visibility takes care of itself. It doesn't — not fully. Google AI Overviews shares real DNA with traditional local SEO, but it makes its own decisions on top.
What AI Overviews actually leans on
Google AI Overviews and AI Mode appear directly on the search results page, ahead of the traditional ranked list, on billions of queries. They lean heavily on Google's own understanding of the local business graph — primarily your Google Business Profile — plus the wider web, your reviews, and schema.org structured data on your site.
There's an important mechanical difference too: there is no public API for AI Overviews. The good news: you're not starting from zero. Several of the signals that make classic local SEO work are the same signals AI Overviews weighs:
- Clean, complete, consistently-formatted Google Business Profile — the anchor for both.
- Structured data (schema.org markup) that makes your services, location, and hours machine-readable.
- Strong, recent reviews with a healthy response rate.
- Consistent name/address/phone (NAP) details across your site and every directory.
If your local SEO fundamentals are genuinely solid, you already have real AI-visibility equity. The gap most med spas have isn't that they did local SEO wrong — it's that AI Overviews demands a layer of machine-readable precision that a page built purely to read well to a human doesn't automatically provide.
Where it genuinely diverges
Traditional local SEO optimizes for a ranked list — you can rank #4 and still get meaningful traffic from someone willing to scroll. AI Overviews and AI Mode return a single synthesized answer that names two or three businesses. There is no #4. Sixty-eight percent of Google searches now end without a click at all, because the AI answer is the destination — the ranked list underneath it is increasingly beside the point for that query.
The other divergence is geographic. Traditional local SEO tends to center on your exact address and the immediate radius Google Maps draws around it. Customers, though, don't only search from inside your city — they search from neighboring suburbs, adjacent zip codes, and transit corridors. A downtown med spa should show up for someone searching from the suburb next door, not just people standing on its block. Tracking that fuller local corridor, not just your exact pin on the map, is a meaningfully different scope than a classic local SEO audit covers.
Keep doing your local SEO fundamentals — they still matter. But treat AI Overviews as its own surface with its own measurement problem (no API, so it has to be scraped) and its own scope (a corridor, not a pin on a map) — not a byproduct you get for free.
